Clinton Hart, FNP-BC
Life can be full of unexpected twists and turns and it’s not always easy. Through faith and family support, I can always find joy in what I’m doing - especially when I’m serving others. I try to approach each participant with compassion and empathy. I hope I can help them live life on their terms – aging well and enjoying a good quality of life.
His medical career began as an emergency medicine nurse at a trauma center hospital in Las Vegas, Nevada. He later moved to California and worked as an emergency department nurse at Riverside Community Hospital and at Loma Linda University Hospital. Clinton also worked as a nurse with a pandemic relief project serving the Los Angeles County homeless population. In addition to his traditional duties as a nurse, Clinton has always sought ways to support new nurses through formal preceptor programs and mentoring opportunities.
Clinton is passionate about his family, which includes his wife, three children and two Chihuahuas. They enjoy spending time at the beach, boating, visiting zoos, aquariums and Disneyland. His personal hobbies include fitness, fishing and recreational target shooting.
Clinton is an associate of Global Transitional Care Medical Group, a Care Partners Company, which provides California-licensed physicians and nurse practitioners to serve participants of InnovAge California PACE (ICAP).
