In-Home Assistance

In-home assistance is included in InnovAge PACE.

In-Home Assessment
One of the first steps in joining PACE is a home safety evaluation to assess fall risks, including loose rugs, electrical cords or clutter. We also look for improvements that could make the home safer and more accessible for you, such as wheelchair ramps or grab bars. Each participant and their caregiver receives safety education after the evaluation.

In-home improvements that are considered medically necessary are provided at no cost to participants — they are part of PACE’s all-inclusive care.

If your care team determines it’s medically necessary for additional in-home help, PACE participants may receive in-home medical care and or personal care.

Medical Care
If medically necessary, PACE participants can receive personalized medical care at home. Care is provided by registered nurses and certified nursing assistants. Care may include:

  • Medication management 
  • Incontinence care and assistance with ostomies and catheters 
  • In-home injections and blood draws 
  • Support after hospitalization

Personal Care
In-home personal care, if medically necessary, is provided by non-medical members of the InnovAge team. Services may include:

  • Medication reminders 
  • Bathing and dressing 
  • Light meal preparation 
  • Light housekeeping

Please note that InnovAge PACE participants must receive all needed healthcare, including primary care and specialist physician services (other than emergency services) from InnovAge or a person or company authorized by InnovAge. InnovAge PACE participants may be fully and personally liable for the costs of any out-of-network services.
