☀️ InnovAge has two PACE centers in the Golden State of California, located in the capital city of Sacramento and in the Inland Empire.

The Sacramento center opened in 2020. The Inland Empire center, serving seniors in Riverside and San Bernardino counties, was remodeled and expanded in 2021.

All InnovAge facilities feature local artwork and photography, bringing a unique and creative feel to each center or administrative building.

InnovAge has six PACE centers in Colorado and its corporate headquarters is located in Denver. Denver Metro area PACE centers are located in Aurora, Denver, and Lakewood. Another center is located in the suburb of Thornton.

In the northern Front Range, InnovAge's Northern Colorado PACE center sits in Loveland and serves the communities of Fort Collins, Loveland, and Greeley. InnovAge's Pueblo PACE center in Southern Colorado opened in 2010 and is housed in a historic bank building in old town Pueblo.

If I join InnovAge PACE, what happens to my health insurance?

Once you join PACE, InnovAge becomes your health insurance provider. InnovAge also coordinates and pays for all of your necessary medical care.

Please note that InnovAge PACE participants must receive all needed healthcare, including primary care and specialist physician services (other than emergency services) from InnovAge or a person or company authorized by InnovAge. InnovAge PACE participants may be fully and personally liable for the costs of any out-of-network services.

What is InnovAge PACE?

InnovAge PACE (Program of All-inclusive Care for the Elderly) is an alternative to nursing facilities. It provides customized healthcare and social engagement to older adults to help them stay independent and in their homes. PACE participants receive most of their healthcare and social support at a PACE center in their community. Each person in the program has a team of medical experts and a personal care plan that is focused on providing a personalized PACE experience. InnovAge is one of more than 100 PACE providers across the U.S.

Who can join InnovAge PACE?

To participate in PACE, you must be

  • At least 55 years old;
  • Eligible for nursing facility level of care; and
  • Living in an InnovAge PACE service area.
