Your PACE Care Plan

If you want personalized care that helps you live independently, InnovAge PACE may be just what you’re looking for.

When you enroll in the PACE program, a team of professionals will work with you and your family (or other caregivers) to develop your customized health plan.

Your plan is developed with input from all members of your care team. They spend time getting to know you, your living situation, your preferences, and your goals. Combined with the results of your initial and periodic health assessments, your care plan is developed when you join PACE and it is updated as needed. 

The goal of PACE is to meet the healthcare needs of seniors through community-based care, keeping you out of institutionalized care for as long as possible. While InnovAge has networks of local speciality providers and facilities, most of your care is provided at your PACE center. This is also where your care is coordinated and where you will meet with members of your care team for assessments and to review your care plan. 

InnovAge's care team partners with you and your loved ones to maintain and improve your health to help you stay independent and at home instead of receiving care at a nursing home or other facility.

Please note that InnovAge PACE participants must receive all needed healthcare, including primary care and specialist physician services (other than emergency services) from InnovAge or a person or company authorized by InnovAge. InnovAge PACE participants may be fully and personally liable for the costs of any unauthorized or out-of-network services.
