Coordinated Healthcare
Coordinated healthcare is the core of PACE. Each participant has a team of medical experts dedicated to providing personalized healthcare. This includes:
- Primary care
- Hospitalization and emergency care
- Specialty care
- Medication management

Transportation
PACE provides transportation to get you from your home to the PACE center and to offsite medical appointments. Our transportation is:
- Wheelchair-accessible
- Unlimited if approved by your care team

Specialty Care
PACE offers participants easy access to dental, hearing, vision, and foot care services. This can include
- Dentures
- Hearing aids
- Eyeglasses

In-Home Assistance
InnovAge provides medical and personal care in the home to help PACE participants stay independent for as long as possible.
